Primary care is the long-term relationship between a person and their clinician. Primary care means that you will have one on one time with the physician that will cover most of your needs. This is relational care that includes coordinating additional health care services when speciality care is needed. Your Primacy Care Provider (PCP) makes sure that those physicians are talking to each other and that someone is circling back with you the patient.
We understand that patients don’t live in our exam rooms. Our care expands to provide patent education on disease management, such as diabetes or heart disease, at free community events and screenings at Health Fairs.
On occasion our services to you may include Hospitalists. Hospitalists are physicians who specialize in the practice of hospital medicine. Hospitalists have more expertise in caring for complicated hospitalized patients on a daily basis. They are also able to meet with family members, follow-up on tests, answer nurses’ questions and deal with problems that may arise.
